Uses and Disclosures of Protected Health Information Your protected health information may be used and disclosed by your physician, our staff, and others who are involved in your treatment.  Below is an explanation.

Treatment: We will use and disclose your protected health information to provide, coordinate, or manage your health care.  For example, your information may be provided to a physician whom you have been referred to.

Payment: Your protected health information will be used and disclosed to obtain payment for services by us.  For example, we may use your information regarding your medical treatment to process claims.

Healthcare operations: We may use or disclose your health information to support the business activities of our office.  For example, your information may be used for quality assessment and improvement activities.

  • You have the right to inspect and obtain a copy your protected health information.
  • You have the right to request a restriction of your protected health information. You may request a restriction by writing a letter to our office.  Optimus Vision Care is not required to agree to the restriction.
  • You have the right to request to receive confidential communications from us by alternative means.
  • You have the right to amend your protected health information.
  • You have the right to receive an accounting of certain disclosures about your protected health information, such as an accounting of mandatory public health reporting.  This right excludes disclosures that you authorized us to make and also treatment, payment, or operations described in this notice.
  • You have the right to revoke this authorization in writing at any time.  Please understand that Optimus Vision Care is unable to take back any disclosures already made with your authorization.
  • You have the right to obtain a paper copy of this notice upon request.

Complaints You may complain to us or to the Secretary of Health and Human Services if you believe your rights have been violated.  You may file a complaint by writing a letter to us.  We will not retaliate against you.

This notice is effective as of July 26th, 2011.  We may change the terms of our notice at any time.  You may request a revised version during your next appointment or by calling our office.
